Position Overview Reporting to the Resource Coordinator, the ISW (Integrated Support Worker) will have knowledge and training in personal care skills and be a certified Health Care Aide (HCA). Preferably, the ISW will also have additional training in working with individuals experiencing mental health or behavioral challenges but additional training may be obtained after start date. The ISW is responsible for client care involving Health Care Aid duties and supportive care duties as directed by the Case Coordinator/Resource Coordinator. This includes partnering with clients to improve their wellness in community, social functioning, self-care, communication, self-help, and coping. ISWs are located in several housing projects that operate in a 24/7 team model, as well as in the community at large. Experience Two (2) years’ experience as an HCA (Home Care Attendant /Health Care Aide) preferred. Two (2) years’ experience working in Mental Health preferred (may include experience as an HCA working with clients with complex needs or behaviors). Education (Degree/Diploma/Certificate) WRHA recognized HCA certificate required. Grade XII minimum or GED equivalent. Certification/Licensure/Registration Valid driver’s license and vehicle required.
